How often does mulch need refreshing?

Rock or mulch?
Mulch for planting beds, because it feeds the soil and holds moisture. Rock for drainage runs, downspout splash zones, slopes and shoreline edges where you want something that stays put.
Do I need landscape fabric?
Under rock, usually yes. Under wood mulch in a planting bed, often no — fabric gets clogged, weeds root on top of it anyway, and it makes future planting a fight.
